- Name and contact information
- State or government-issued identification
- Employment verification letter
2. Information to verify the organization
- Organization's name and contact information
- Organization license number, 501c3 number, or TIN
3. Information about the organization and the types of services it provides
- Organization description
- Approved services
- School grades and regions served
- Organization logo
