What information should I include in my invoices or receipts?

A receipt is necessary when a service or good has been paid for and the parent/guardian would like to be reimbursed. 

An invoice is necessary when a service or good has not been paid for and the parent/guardian would like the service provider to be reimbursed.  

The receipts or invoices that are uploaded in the claim process should contain the following information: 

  • The name of the service provider 
  • Complete name and address of person that purchased the service or good 
  • Where possible, the name of the child should always be included
  • The date the service occurred or will occur
    • Date ranges are not acceptable
    • Effective September 15, 2023, all invoices for tutoring must include specific dates of service on all invoices.
    • Effective September 15, 2023, invoicing for tutoring is limited to no more than a month (30 days) in advance. 
    • Effective January 5, 2024, all invoices for Before- or after-school educational programs must include specific dates of service on all invoices.
    • Effective January 5, 2024, invoicing for Before- or after-school educational programs is limited to no more than a month (30 days) in advance.
  • The date the good was purchased or invoiced
    • Eligible goods may be purchased by homeschool families only
  • The date the service was paid for or invoiced
  • The total amount paid or invoiced
  • A statement attesting that the parent/guardian has reviewed and approved
    the invoice being submitted.
