QESP Application Requirements

This list is updated as of 7/1/2024 and is subject to change or be updated for clarity.

General Requirements:

  • The business license is aligned with services
  • Price list of declared services
    • Lowest price cannot be $0 or pricing exclusive to ACE students
  • *Sample Invoice/Receipt 
    • *Required for all organization types except Sole Proprietor
  • A website with listed declared services/goods
  • The location where services are provided (must be commercial address)

  • Service agreement with refund policy.
    • Service Agreement examples include:  Cancelation policy, hours of operation, refund policy, payment schedule, important dates, privacy policy
  • Policy Handbook (may exist in a single document)
    • Policy Handbook examples include: Dress code, Code of conduct, the number of students attending, behavioral expectations, medical forms, transportation policy
  • Description of the educational value of services

*Important: Invoices must meet requirements to be considered eligible for claim approval. (review requirements)

Day Camps or Before and After School Activities:

  • Verification of Applicant as Admin
  • Registration form
  • Day Camp Schedule/ itinerary (listing educational content)
  • Lesson Plans
    • Day Camps do not need lesson plans but must show academic alignment.
    • Sample Lesson Plan must include, at a minimum: 
      • An objective/standard
      • Activity
      • Assessment of understanding of a subject.
  • Location:
    • Day Camps/Before and Afterschool activities cannot be online/virtual
    • Cannot be a private residence. 
  • Professional credentials that show your ability to provide the services your organization offers. For example, a degree, professional license, resume with relevant job skills & references, or a related certification. 
    • Demonstrate qualification with a degree, certificate, educational credits, or relevant work experience to conduct an educational program.

Field Trips (Approved museums, theaters, science centers, historical landmarks):

ACE is no longer permitting tour/transportation companies to appear in the ACE Marketplace; however, schools or other approved providers may organize and facilitate allowable field trips. 

  • Program Schedule, including dates and location
    • Cannot be a private residence. 
  • Price list for admission (may be on the website)

Language Classes:

  • Lesson plan
    • Sample Lesson Plan must include, at a minimum:   
      • An objective/standard 
      • Activity 
      • Assessment of understanding of a subject.
  • Sample work completed by the student. Please do not include personally identifiable information.
  • Professional credentials that show your ability to provide the services your organization offers. For example, a degree, professional license, resume with relevant job skills, or certification.
  • Location cannot be a private residence

Instrument Lessons:

  • Registration form
  • Lesson plan
    • Sample Lesson Plan must include, at a minimum:   
      • An objective/standard 
      • Activity 
      • Assessment of understanding of a subject.
  • Professional credentials that show your ability to provide the services your organization offers. For example, a degree, professional license, resume with relevant job skills, or certification.
  • Location cannot be a private residence

Tutoring

  • Subject areas offered
  • Grades qualified (or licensed) to Tutor
  • Lesson plan
    • Sample Lesson Plan must include, at a minimum:   
      • An objective/standard 
      • Activity 
      • Assessment of understanding of a subject.
  • Sample work completed by the student. Please, no personally identifiable information.
  • Professional credentials that show your ability to provide the services your organization offers, such as a degree, professional license, resume with relevant job skills, or certification.
  • Location cannot be a private residence

Educational Learning and Study Skills:

  • Specific study skills included (i.e. critical reading skills, reading, comprehension, note-taking using sources of information, time management, and memorization)
  • Lesson plan
    • Sample Lesson Plan must include, at a minimum:   
      • - An objective/standard 
      • - Activity 
      • - Assessment of understanding of a subject.
  • Sample work completed by the student  (Please, no personally identifiable information.)
  • Professional credentials that show your ability to provide the services your organization offers, such as a degree, professional license, resume with relevant job skills, or certification.
  • Location cannot be a private residence

Curriculum and Material (*Homeschool only):

  • *Tuition is not an allowable expense*
  • Location cannot be a private residence
